One of the top questions asked of public law librarians is the forms question ("Is there a form to do x?"). Oregon does not have many standard court forms; so the answer to the forms question is rarely "yes." The following are a few resources useful in locating court forms:
The Oregon Judicial Department has a Forms Center, but you’ll want to check your local court’s website to see if there are forms specific to your court.
Court-approved forms also appear in some court rules, such as the Oregon Supplementary Local Rules and Uniform Trial Court Rules.

Some vendors sell legal forms online. It is imporant to examine the source and ask questions like, is the company well known; do they have Oregon specific forms; do they offer instructions?
